Personal Support Services

Our Community and Personal Support program is designed to support those in need, live safely at home. The services are accessible by members of Dilico Anishinabek Family Care’s health transfer communities, those in assisted living, and for individuals funded through Jordan’s Principle.

Our in-home services include, but are not limited to: personal care; assistance with medication reminders; respite for caregivers; health monitoring including assistance with self-monitoring of blood glucose and home blood pressure monitoring; support with wheelchair accessible transportation for errands or medical appointments; food preparation; assisting clients with range of motion exercises that have been recommended by Physical Therapy/Occupational Therapy; assistance with transferring using lift mechanisms; assisting client with refilling oxygen tanks; community support during times of crisis such as community evacuation to Thunder Bay; community support with Adult Life Enrichment programming; basic foot care/wound care; socialization for elders; light housekeeping; supporting children/adolescents for recreational outings; and client advocacy to allied health professionals such as nurses, primary care providers, speech and language therapists, and physiotherapists/occupational therapists.
