Home and Community Care

Dilico Home and Community Care (HCC) provide a range of in-home healthcare and support services to Anishinabek people who have acute, chronic, rehabilitative or palliative care needs, and are living in First Nations or in the City of Thunder Bay.

Services are available on an occasional or ongoing basis and are best suited for people who need help to live independently in their own homes because of short or long-term illness, physical challenge or aging. Our Care Managers visit children, youth and families at home to provide clinical assessments that determine which services accommodate each person’s unique needs.

Nursing (Acute and Chronic Care)

Discharge Planning

Physiotherapy

Occupational Therapy

Social Work

Speech and Language Pathology

Nutritional and Dietary Counselling

School Health Support

Personal Support

Care Coordination

Who Can Use Home and Community Care Services?

Individuals are eligible for this service if it is determined that they require professional health services, personal care, adult life enrichment, school health support services or any other community services.

Individuals from any of the following First Nations may be eligible for Dilico’s Home and Community Care Services: Fort William, Lake Helen, Whitesand, Long Lake #58, Ginoogaming, Pays Plat, Pic Mobert, Michipicoten and Biinjitiwaabik Zaaging Anishinaabek.
