The Care Manager is the primary person responsible for assisting clients and their support systems in maintaining the highest possible level of independence and functioning in the least restrictive environment within a rural community setting.
Qualifications and Requirements: A Registered Nurse (RN) diploma is required. A four year BScN degree is preferred. Three (3) years direct experience in the delivery and/or coordination of home-care services; demonstrated sound clinical knowledge and experience in home-care case management and/or community based home-care service delivery. Candidates without the specific education qualifications and where the position does not require certified qualification to practice, but who possess an appropriate combination of experience and other academic qualifications will also be considered.
Hours of Work: 32.5 hours per week
Must have: Must be willing and able to work flexible hours and provide on-call services as required; must be certified in CPR and Standard First Aid; must provide evidence of registration with the appropriate regulatory bodies (College of Nurses and Registered Nurses Association of Ontario); he/she must be able to travel regularly, have a valid Ontario Driver’s License and have access to a personal vehicle and $1,000,000 liability; a satisfactory criminal records/vulnerable sector check is required upon hiring.
