Location: Long Lake First Nation #58

The Care Manager is the primary person responsible for assisting clients and their support systems in maintaining the highest possible level of independence and functioning in the least restrictive environment within a rural community setting.
Qualifications and Requirements: A Registered Nurse (RN) diploma is required. A four year BScN degree is preferred. Three (3) years direct experience in the delivery and/or coordination of home-care services; demonstrated sound clinical knowledge and experience in home-care case management and/or community based home-care service delivery; candidates without the specific education qualifications and where the position does not require certified qualification to practice, but who possess an appropriate combination of experience and other academic qualifications will also be considered.
Must have: The ability to work flexible hours; travel out of town periodically, a valid Class “G” Driver’s License; access to a reliable, personal vehicle; $1,000,000 liability vehicle insurance, must provide evidence of registration with the appropriate regulatory body; and must have a Standard First Aid/CPR certificate; a satisfactory criminal records/vulnerable sector check is required upon hiring.
Hours of Work: 32.5 hours per week
