The Care Manager is the primary person responsible for assisting clients and their support systems in maintaining the highest possible level of independence and functioning in the least restrictive environment within a rural community setting.
Qualifications and Requirements: A RN Diploma is required. A four-year BScN is preferred. Three (3) years direct experience in the delivery and/or coordination of home care services; demonstrated sound clinical knowledge and experience in home care case management and/or community based home care service delivery; and a combination of education and work experience pertaining to community based health care services may be considered.
Hours of Work: 25 hours per week
Must have: The ability to work flexible hours; travel out of town periodically, a valid Class “G” Driver’s License; access to a reliable, personal vehicle; $1,000,000 liability vehicle insurance, must provide evidence of registration with the appropriate regulatory body; and must have a Standard First Aid/CPR certificate.
